Late Complications After Previous Surgical Repair for Coarctation With Extra-Anatomic Bypass Graft: Report on Two Cases

نویسندگان

چکیده

Surgical correction of aortic coarctation (CoA) has been the standard treatment in infants and adolescents to prevent late consequences related obstruction distal hypoperfusion. Several surgical techniques for CoA repair have traditionally applied. However, complications, including re-CoA pseudoaneurysm formation, are not uncommon. The incidence complications is highly type initial surgery. Here, we reporting two cases an infrequently used technique, extra-anatomical bypass graft (EABG). first case presented with at anastomosis site descending aorta treated by endovascular stent graft. second stenosis proximal left subclavian artery (LSCA) medically upon request patient.

Endovascular repair of a late pseudoaneurysm after an extra-anatomic bypass for aortic coarctation.

Figures 1 and 2 describe the management of a 47-year old patient presenting with a chronic false aneurysm after an extra-anatomic bypass placed for aortic coarctation during childhood. The condition was treated with two covered stents of decreased diameters and an hourglass-shaped plug. Post-procedure and control imaging at 1 year showed no complications. Figure 1: Preoperative findings. (A) Th...

Single Stage Repair for Aortic Coarctation associated with Intracardiac Defects Using Extra-Anatomic Bypass Graft in Adults

BACKGROUND AND OBJECTIVES Coarctation of the aorta in adulthood is generally associated with other cardiovascular disorders requiring surgical management. An extra anatomic bypass grafting from the ascending to descending aorta by posterior pericardial approach via median sternotomy could be a reasonable single stage surgical strategy for these patients. SUBJECTS AND METHODS Seven male patien...

Patent Extra-Anatomic Graft 12 Years after Ascending-Descending Aortic Bypass for Interpositional Graft Infection

A male car driver who sustained polytrauma in a road traffic accident when he was 25 years old, had emergency partial hepatectomy performed for liver laceration, followed by interpositional graft (18 mm woven Dacron) repair for transection of the proximal descending aorta via a left thoracotomy. He also had multiple fractures which were treated conservatively. His postoperative recovery was com...

Use of apical suction to facilitate extra-anatomic bypass for recurrent coarctation: a case report

The use of apical suction devices has been well described for maintaining satisfactory haemodynamics during off-pump surgical coronary revascularization. Its expanded use has been described in a few other situations. We describe here a case of recurrent coarctation where an extra-anatomic ascending to descending thoracic aorta bypass graft was constructed using cardiopulmonary bypass without ar...

Long-term results of endovascular aortic repair for thoracic pseudoaneurysms after previous surgical coarctation repair.

OBJECTIVES Late aneurysm formation has been reported after every type of surgical coarctation repair in up to 10%, with rupture of such aneurysms being responsible for approximately 7% of all deaths. Secondary surgical repair carries a significant mortality (up to 15%) and morbidity rate (recurrent laryngeal nerve paralysis ∼20%, phrenic nerve injury ∼5%). According to the positive experience w...
